It is the benign enlargement of breast tissue in males. The condition may occur physiologically in newborns due to transfer of maternal or placental estrogens from the mother or at any time from adolescence onwards. Gynecomastia surgery may arise as an abnormal condition associated with disease or metabolic disorders, as a side-effect of medication, or as a result of the natural decrease of testosterone production in older males.

Treatment and Male Breast Reduction Surgery:

For men who feel self-conscious about their appearance, male breast reduction surgery is an ideal option to flatten the breast area.

Male breast reduction surgery usually takes around 90 minutes. The surgeon will make an incision around the areola and liposuction may be used to suck out excess fatty tissue. If there is a lot of tissue to remove, cuts may extend down the chest from the areola and the nipples may need to be repositioned. Results are permanent but weight gain, hormonal imbalances and the use of certain drugs can cause the breast area to enlarge again.

Ideally, the male chest is muscular and well defined. It signifies strength and masculinity. Sometimes, for various reasons, the male breast becomes fatty and begins to sag. Instead of a healthy masculine look, the man is left with an unhealthy, out of shape, feminine look. Gynecomastia (Enlarged Breasts in Male) is the abnormal development of large mammary glands & excessive fat deposits in males resulting in breast enlargement. The condition can occur physiologically in neonates (due to female hormones from the mother), in adolescence, and in the elderly. In adolescence and elderly, it can be an abnormal condition associated with disease or metabolic disorders, or it can be due to generalized obesity. In adolescent boys the condition is often a source of distress, but for the large majority of boys whose pubescent gynecomastia is not due to obesity, the breast development shrinks or disappears within a couple of years in gynecomastia surgery

The causes of common gynecomastia remain uncertain, although it has generally been attributed to an imbalance of sex hormones or the tissue responsiveness to them; a root cause is rarely determined for individual cases. Approximately 5-10% cases of gynecomastia are due to drugs specially steroids, aldactone (spironolactone) or some cardiac drugs like digoxin. Un-prescribed steroid intake by bodybuilders for rapid development of muscular mass is now a day commonly seen in this group of patients. Breast prominence can result from hypertrophy of breast tissue, chest adipose tissue (fat) and excessive skin, and is typically a combination.

The good news is that with modern plastic surgery, gynecomastia can be effectively treated so that you no longer have to be embarrassed to take your shirt off. The treatment of gynecomastia surgery depends on what is causing the problem. The three components of gynecomastia are

·         Excess fat,
·         Excess breast tissue, and
·         Excess skin.

Carpal tunnel release surgery in India

India offers great advantages to people looking for affordable carpal tunnel syndrome (release) surgery as not only the cost of carpal tunnel syndrome surgery in India is quite low but the orthopedic surgeons in India are well accomplished in the field, hospitals in India are offer latest and most advanced technology and there is no waiting time for the procedure in India.

Carpal Tunnel Syndrome

Carpal Tunnel Syndrome occurs when the median nerve which runs through the wrist to the hand becomes compressed in carpal tunnel release surgery. The problem involves the narrow bony passage in the wrist called the carpal tunnel, through which nine tendons and the median nerve must pass between the arm and hand.

In the wrist, the median nerve (the major "communication line" between the brain and the hand) is routed through a small passage bounded by the wrist bones (the carpals) and the tough ligament tissue connecting them (see diagram). Nine hard tendons also pass through this crowded "carpal tunnel". Because this structure is rigid, any influence that causes tissue swelling within the tunnel or reduces its size tends to compress and pinch the median nerve.

Symptoms of Carpal Tunnel Syndrome
  • Numbness and tingling sensation in one or both hands.
  • Frequently accompanied by sharp pains radiating through the arm or shoulder.
  • Muscle weakness of index and middle fingers and thumb.
  • Discomfort / aching in one or both hands.
  • Lack of feeling in hand may cause person to drop objects.
  • Activities requiring fine hand motor skills may prove difficult if not impossible.
 Treatment of Carpal Tunnel Syndrome

When symptoms of carpal tunnel syndrome are minor or occur only during sleep, relatively simple measures to minimize swelling or otherwise limit median nerve compression may provide effective relief. Such efforts might include splint to keep the wrist in a non-aggravating position, cortisone injections to relieve wrist swelling, diuretics (water pills) to reduce fluid accumulation, vitamin B tablets and anti-inflammatory medications in carpal tunnel release surgery.

Carpal Tunnel Release Surgery

When these initial efforts fail to control symptoms, an operation called a carpal tunnel releasesurgery is advised by your surgeon. During the procedure, the surgeon carefully cuts the wrist ligament, releasing the pressure inside the carpal tunnel, usually providing almost instantaneous relief from arm and hand pain and tingling.

As the wound heals, new tissue will bridge the severed ligament in about six weeks, making the ligament long enough to create additional space inside the tunnel and prevent recurrence of symptoms in most cases.

This surgery is usually done on an outpatient basis. Complete recovery can take anywhere from several weeks to a year, depending on the severity of damage to the nerve.

What Is Urethral Strictures?

Urethral stricture refers to any narrowing of the urethra for any reason whether or not it actually impacts the flow of urine out of the bladder. Any inflammation of urethra can result in scarring, which then can lead to a stricture or a narrowing of the urethra in treatment of urethral strictures. Trauma, infection, tumours, surgeries, or any other cause of scarring may lead to urethral narrowing or stricture. Mechanical narrowing of the urethra without scar formation (developmental causes or prostate enlargement) can also narrow the urethra. Corrective treatment is only necessary if your stricture causes problems. You'll need frequent follow-up exams for at least a year after treatment to ensure the stricture doesn't recur and that you remain free of infection.

What is the Treatment of Urethral Strictures?

Catheterization

Inserting a small tube (catheter) into your bladder to drain urine and relieve pain — in the case of a complete blockage — is the usual first step in treatment. Self-catheterization is an option if you're diagnosed with a short stricture in treatment of urethral strictures. Simply inserting a catheter may correct and open the urethra, if done periodically.

 Urethroplasty

Urethroplasty surgically removes the narrowed section of your urethra or enlarges it. The recurrence of strictures after urethroplasty is low.

 Endoscopic internal urethrotomy

Your doctor inserts a cystoscope a thin optical device into your urethra. Instruments inserted through the cystoscope into the urethra remove the stricture or vaporize it with a laser. This surgical procedure offers faster recovery, minimal scarring and less risk of infection, although recurrence is possible.

 Dilation

Your doctor inserts a tiny wire through the urethra and into the bladder. Progressively larger dilators pass over the wire to gradually increase the size of the opening. This outpatient procedure may be an option if you have recurrent strictures in treatment of urethral strictures.

Implanted stent or permanent catheter. If you have a severe stricture and choose not to have surgery, you may opt for a permanent artificial tube (stent) to keep the urethra open, or a permanent catheter to drain the bladder. However, these procedures have several disadvantages, including risk of bladder irritation, discomfort and urinary tract infections. They also require close monitoring.
